Questions to ask
Use the same questions whether you are using insurance or asking about a self-pay option.
- What service is the estimate for, and what is included?
- Could a facility, laboratory, imaging, medication, or device charge be billed separately?
- Does my plan require prior authorization or a specific site of service?
- When could the estimate change, and whom should I call with a billing question?

An estimate is not a treatment recommendation
Clinical candidacy is determined through evaluation. Do not choose a procedure based on price alone, and do not assume that a service is appropriate or available until the clinical and scheduling teams confirm it.

Confirm insurance and self-pay details separately
Insurance participation and benefits can change. A self-pay estimate may follow different terms. Ask the office which pathway applies and request the current information for the exact service being considered.
