Call the practice or use the appointment-request guide to identify the right starting point.
- Confirm whether your plan requires a referral
- Ask which prior pain records, discharge letters, or imaging reports are needed
- Verify the clinic location and arrival time
- Keep private health information inside the confirmed secure records or paperwork route

What to bring
Unless the office gives you different instructions, it is useful to have the following available:
- Photo identification and a current insurance card
- An up-to-date medication and allergy list
- Relevant imaging reports and prior treatment records requested by the office
- A short timeline of symptoms, what changes them, and how they affect daily activity
- Questions you want answered and the activities you hope to regain or protect

What the first evaluation is for
The first visit is used to understand the history, examine the pain pattern, review relevant information, and decide what additional evaluation or care may make sense. Do not assume that a procedure will occur at the first appointment.
