How do I book an appointment?

Online appointment booking is temporarily paused while Integrity prepares Athenahealth self-scheduling for an October launch. Call your clinic for scheduling assistance.

Do I need a referral?

Referral requirements can depend on your insurance plan and the type of visit. Call the office before scheduling so the team can tell you what is needed for your situation. If another clinician referred you, ask that office to send relevant records and imaging reports.

Does Integrity accept my insurance?

Plan participation and benefits can change. Contact the office with your current insurance information so the team can check participation and help you understand any authorization or referral requirements. Benefit verification is not a guarantee that an insurer will pay a claim.

Where are the clinics?

Integrity has public clinic locations in Scottsdale, Mesa, Glendale, and Prescott. Visit the locations page for current addresses, posted hours, and directions. Confirm the location shown on your appointment before traveling.

How do I complete new-patient or follow-up paperwork?

Use the secure patient-form experience on this site. Your information is entered directly into the DashQuill form system and is not received or stored by this website. .

What should I bring to an evaluation?

Bring a photo ID, current insurance card, an up-to-date medication list, and any records or imaging the office asked you to provide. It can also help to write down when symptoms began, what changes them, prior treatments, and the activities you want to return to.

What happens during a pain evaluation?

A typical evaluation starts with your history: where symptoms occur, when they began, what affects them, and how they limit daily life. The clinician may perform a focused physical or neurologic examination and review prior records or imaging. Testing or treatment is discussed only when it may help clarify or address the working diagnosis.

Will I receive a procedure at my first visit?

Do not assume a procedure will happen at the first evaluation. The clinician first needs to understand the symptoms, diagnosis, prior care, medications, and health risks. Some treatments also require records, insurance authorization, preparation, or a separately scheduled visit.

How should I prepare for a procedure?

Preparation varies by procedure and by your health history. Follow the instructions provided specifically for your appointment. Before changing blood thinners, diabetes medicines, or any other prescription, speak with the clinician who manages that medication and the Integrity team. If instructions are unclear, call the office before the procedure.

What should I expect after a procedure?

Recovery, activity limits, and the time needed to assess a response differ by procedure. Use the written instructions given to you, arrange transportation if directed, and know which symptoms should prompt a call. The treatment guides provide general context but do not replace your own instructions.

When to contact the office

When should I call before an appointment or procedure?

Call if you develop a fever or new illness, have a change in medications, start an antibiotic or blood thinner, may be pregnant, cannot follow the preparation instructions, or are unsure where or when to arrive. The team can tell you whether the visit should proceed or be rescheduled.

What if symptoms become urgent?

Patient forms, portal messages, and appointment requests are not monitored for emergencies. Call 911 or go to the nearest emergency department for severe or rapidly worsening symptoms, trouble breathing, signs of stroke, loss of consciousness, major injury, or another possible medical emergency.
